CLINICAL CONSULTATIONS

All treatments require a consultation and clinical suitability assessment before treatment can proceed.

Revitalise Plus reserves the right to refuse treatment where it is considered clinically inappropriate or unsafe.

MEDICAL DISCLOSURE

Clients must provide accurate and complete medical information during consultations, including:

Pregnancy or breastfeeding status

IV NUTRIENT THERAPY (IVNT)

Intravita IV Nutrient Therapy treatments are designed to support hydration, energy, immunity, recovery and overall wellbeing.

All IV infusions are administered by Sarah, Nurse Practitioner and Independent Nurse Prescriber, within a professional clinical environment following a consultation and nursing assessment.

Each appointment includes clinical observations, assessment and aftercare advice.

INTRAVENOUS MICRONUTRIENT SUPPLEMENTATION (IMS)

Intravenous Micronutrient Supplementation (IMS) is designed to support optimum health and wellbeing. It is not intended to diagnose medical conditions or replace conventional medical care.

Results may vary between individuals and no guarantees can be provided regarding treatment outcomes.

Existing medical conditions, medications and lifestyle factors may impact results. Clients are responsible for informing the practitioner of any medications or health conditions prior to treatment.

Clients under the care of a GP, Consultant or other healthcare provider are encouraged to keep their healthcare professionals informed regarding nutritional supplementation and IV therapy.

APPOINTMENTS & CANCELLATIONS

Appointments may require advance booking and confirmation.

We kindly request sufficient notice for cancellations or rescheduling. Failure to attend appointments or repeated late cancellations may result in charges or refusal of future bookings.

PAYMENT TERMS

Payment is due at the time of treatment unless otherwise agreed.

Prices and services may change without notice.

RESULTS & LIABILITY

Treatment outcomes vary from person to person. Revitalise Plus cannot guarantee specific results from any treatment or programme.
